How To Use the Patient Portal
If you provided an email address at time of scheduling your appointment, an email will be sent to you titled “Patient Portal” from welcome@practicemailer.com. If you do not see the email, please check your spam folder. The “Patient Portal” email will contain a link to fill out your health information, upload insurance cards, and other info prior to your appointment.
Contact our office if you are still unable to locate the message, or if you’d like to provide us an email address to access the portal.
Once in the “Patient Portal” email, click the link, then sign in with your date of birth and last name. It will require you to choose a password. The link is valid for 72 hours. If you access it after that time, click the practice url (performancepodiatry.ema.md) and select the purple hyperlink titled “patient portal”, then click “forgot password”. It will then have you enter your email, date of birth, and last name.
Once in the portal, click on “My Health” on the left side of the top banner. It has a heart icon next to it. Then you can go through the menu items on the left-hand side to fill in your address, preferred contact information, health history and more. You can also upload your insurance cards, update your address and more from the tabs on the left hand side.
Please contact our office if you have any questions or troubles. It’s no problem if you are unable to complete your information ahead of time, or prefer to fill out on the day of your appointment. Plan to arrive 5-10 minutes before your appointment time and we will collect it then.
